COVID-19 Vaccine Booking Information

London Public Library Partnership with Middlesex London Health Unit (MLHU)

If you're over 70 and have a library card, Library staff will be calling you to offer help booking a COVID-19 vaccine appointment using the MLHU COVID-19 vaccine booking portal. It may take some time for us to reach every Library cardholder. We appreciate your patience.
You don't have to wait for our call! If you're eligible, you can book your COVID-19 vaccine appointment here:
New appointments open each day at 8am.

Already have an appointment booked and have questions?

Please call +1 (226) 289-3560 (8:00 a.m. to 7:30 p.m., Monday to Sunday) if you:

  • Need to cancel or change your appointment.
  • Did not receive an email confirming your COVID-19 vaccine appointment.
  • Did not receive an email receipt for your first-dose or second-dose of vaccine.
  • Did not receive your second-dose appointment information.
Current Eligibility by Age

Anyone 60+ - is eligible to book appointment for vaccine (Moderna or Pfizer) at a MLHU mass vaccination clinics

Anyone 40+ - is eligible to book appointment for AstraZeneca vaccine in pharmacies

How to book an appointment using the London Middlesex Booking portal online

  1. Go to London Middlesex Booking portal New appointments open each day at 8am
  2. Scroll down to the Age-based criteria box
  3. Enter your date of birth (month/date/year). Click Continue – this will bring you to the booking software, it may take a bit of time to load.
  4. Select the location. There are currently five COVID-19 vaccine clinic locations that you can choose from:
    • Western Fair Agriplex (London)
    • North London Optimist Community Centre (London)
    • Caradoc Community Centre (Mount Brydges)
    • St. Thomas-Elgin Memorial Arena
    • Goff Hall-Woodstock Community Complex
  5. Under the What priority area do you belong to? dropdown menu, select Targeted population
  6. Under the Reason for Immunization dropdown menu, select Age Priority Population>
  7. Click beside I’m not a robot and do the task that it asks (if applicable)
  8. The box will turn to a checkmark and Choose date button will appear – click Choose date
  9. Select a date: Note the comments at the bottom of each calendar day selected will tell you if there are appointments available:
    • There are a few appointments available
    • No appointments are currently available for this day- please select a different day
    • There are many appointments available
  10. Select Choose time
  11. Select the time that you want from available times and select Confirm Booking
  12. Fill in the required information. Please note that only First and last name, date of birth and gender are required. Healthcard number is not required.
  13. Select the check box that confirms that you are eligible and verified.
  14. If you would like to receive information by email or text, select the appropriate boxes (the email and cell phone number boxes appear as you select those options). Note: Email and phone number are not required to book.
  15. Enter email address and cell number if these boxes appear
  16. Click Confirm Selection
