Who Will Speak for You? Planning for Future Care

What would happen if you couldn't speak for yourself about your healthcare? Advance Care Planning (ACP) helps you think about and share your wishes and preferences for future healthcare. In this session, you’ll learn what Advance Care Planning means in Ontario and why having conversations about your wishes can make things easier for you and the people you care about. We’ll explore who can make healthcare decisions for you if you can't make them yourself, what a Substitute Decision-Maker (SDM) is, and the difference between having a Substitute Decision-Maker and naming someone through a Power of Attorney for Personal Care. Using an Advance Care Planning workbook, you'll explore these important questions and prepare for conversations with the people who matter to you.
